The immigration medical exam is a mandatory health screening required by the U.S. Citizenship and Immigration Services (USCIS) for individuals applying for U.S. permanent residency (Green Card).
The purpose is to ensure that applicants do not have any medical conditions that could affect public health or make them inadmissible under immigration law.
At Immigration MD, these exams are performed exclusively by Dr. Allen Smolenski, a USCIS-designated Civil Surgeon, right here in Myrtle Beach.
Our goal is to make the process simple, fast, and stress-free, with clear guidance from start to finish.
Call, text, or book online. You’ll receive a confirmation with instructions on what to bring (passport, vaccination record, I-693 form, etc.).
💡 Tip: Print the most recent USCIS Form I-693 but do not sign it until instructed by Dr. Smolenski.
During check-in, you’ll fill out a short medical questionnaire about your past illnesses, surgeries, and vaccinations.
Dr. Smolenski will perform a comprehensive physical exam following USCIS standards.
This includes:
General health evaluation
Heart, lungs, eyes, and ears check
Mental health screening
Standard tests (required for all adult applicants) include:
Tuberculosis screening (blood test or chest X-ray if indicated)
Syphilis (RPR blood test)
Gonorrhea (urine test)
Your vaccination record will be reviewed to confirm all required immunizations are up to date. Missing vaccines can be administered on site
Once your labs are complete, Dr. Smolenski will sign and seal your USCIS Form I-693.
You’ll receive two copies:
Sealed envelope – must be submitted to USCIS (do not open).
Personal copy – for your records.
The following vaccinations are typically required for immigration applicants. Requirements vary by age and health status:
MMR (Measles, Mumps, Rubella)
Tdap (Tetanus, Diphtheria, Pertussis)
Varicella (Chickenpox)
Influenza
COVID-19
Hepatitis B
Polio
Prevents viral infections common in childhood.
Required for most adults.
Proof of immunity or vaccination required.
Required during flu season (Oct – Mar).
Full vaccination required unless exempt.
For applicants under 18 or as indicated.
For children or unvaccinated adults.
Most applicants receive results within 3–5 business days depending on lab turnaround.
